Seeking further help – request process

If you would like further help with your muscle joint or bone (musculoskeletal) concern, you can contact the Health Board for help using the request for help form below. If you are unable to complete this form electronically and would like assistance, please contact 0300 3732539 (Monday to Friday) and one of our team will help you.

You do not need to visit your GP to access services.

The ‘request for help’ form gives you the opportunity to tell us about how your muscle, joint or bone concern is affecting you and what you would like help with.  

You will also be asked for information about your general health.

This ‘request for help’ form asks you to think about how your bone, muscle or joint problem is affecting you. The information you provide will help us to understand what matters to you. What you tell us about yourself will help us to understand what you need from us as health or care teams as part of rehabilitation and connect you with the right health professional to support you.

Please note that the ‘request for help’ form does not replace medical advice for emergencies. 

If you are an NHS Wales member of staff whose attendance at work is affected by your bone, muscle, or joint problem, please document this in the form.

If you are an Armed Services veteran and your bone, muscle, or joint concern is related to your service, please explain that to us within the form.

What happens to my request for help form?

The team of administrators and therapists who work in the ‘MSK hub’ will read the information you have provided about yourself.  With your agreement, the clinicians will also look at your electronic health records and previous investigations to gain a full picture of your concern and your needs.

It is sometimes necessary for the administrative team or clinicians to contact you to ask you further questions.  This will be to help them understand how best to support you.

There are a number of different ways that you can be supported to manage your muscle, joint or bone concern.  Below are some examples of the types of help that can be offered:

  • Provide the right information and advice so that you can confidently manage your concern yourself
  • Provide information on community support groups that you may wish to connect with
  • Provide information on community-based activity/exercise groups that you may wish to attend
  • Referral to a community based rehabilitation team
  • Referral to the falls service
  • Referral to a hospital based team (e.g. Rheumatology, Orthopaedics, Radiology)
  • Referral to physiotherapy, podiatry, weight management or dietetic services

Before you complete the request for help, some questions for you:

Rehabilitation is more successful when working towards specific activity or targets – we refer to these as ‘goals’. What are your goals? What do you think you need help with to reach these goals?

When thinking about your goals it may be helpful to consider what has changed or been affected as a result of your muscle, joint or bone concern.

You may wish to think about this from the point of view of:

  • Your wellbeing
  • Your daily activities
  • Your work
  • Your relationships
